Jaclyn Yared
FOUNDER
Jaclyn Yared, Owner of Transitions Reintegration Services, brings with her 20 years of experience working in the motor vehicle accident rehabilitation field. A graduate of Grand Valley State University with a Bachelor of Science in Psychology and Behavioral Science, Certified Brain Injury Specialist, Certified Employment Specialist and Certified Professional Career Coach. Jaclyn has always had a passion of helping clients return to work following an accident. Her history of high-placement success rates and repeated referrals are a testament to her dedication and approach to assisting the rehabilitation efforts of those in her care. Jaclyn’s focus now is to continue growing the business in order to further assist individuals across the state of Michigan.

Alex Hogue, MSOT, OTRL
OCCUPATIONAL THERAPIST & VOCATIONAL SPECIALIST
Alex joined Transitions in May of 2021 as an Occupational Therapist and Vocational Specialist. Ms. Hogue obtained a Master’s in Occupational Therapy from Saginaw State University in 2019. She previously worked as an OT in a skilled nursing rehabilitation facility where she gained experience in planning, coordinating and directing the implementation of client-centered, skilled treatment. She has also worked as a Private Caregiver where she provided assistance with ADL’s, daily activities, leisure and work skills. Alex has worked with a wide-range of clients including individuals with dementia and individuals involved in motor vehicle accidents with conditions such as traumatic brain injuries and spinal cord injuries. Overall, Alex is an exceptional addition to our team. She is skilled in building rapport with clients and assisting them with creating and executing functional treatment plans.

Ethan Rogers, MSOT, OTRL
OCCUPATIONAL THERAPIST & VOCATIONAL SPECIALIST
Ethan joined Transitions Reintegration Services in December, 2024. A graduate of Wayne State University with a Bachelor of Science in Biology (2016) and Western Michigan University with a Master’s in Occupational Therapy (2020), Ethan brings experience in both clinical and home health settings.
Before becoming an OT, Ethan dedicated several years to working with children with various disabilities, including autism, blindness, cerebral palsy, and Down's Syndrome. After completing his degree, he helped develop a therapy program at an assisted living facility, where he worked closely with residents to promote engagement and independence. Since then, Ethan has worked in home health care, providing occupational, recreational, and vocational therapy services to individuals recovering from a variety of injuries and conditions. He has extensive experience working with clients from diverse cultural and religious backgrounds, tailoring therapy approaches to meet each individual's needs.

Holly VanHoose, MS, CTRS, CESP, AFP
RECREATIONAL & VOCATIONAL SUPERVISOR
Holly is certified in Recreational, Aquatic & Vocational Therapy services. She has been a Certified Therapeutic Recreation Specialist since 2014 & has been working for Transitions Reintegration Services since 2018. Holly graduated from Central Michigan University in 2013 with a Bachelor of Applied Arts in Recreation, Parks and Leisure Services Administration with a concentration in Therapeutic Recreation, in addition to being awarded a Master of Science in Parks, Recreation, & Tourism Management with a concentration in Recreational Therapy from Clemson University in 2018. Holly furthered her skills by obtaining the Aquatics Fitness Professional certification in 2022 & was awarded the Certified Employment Support Professional designation from the Association of People Supporting Employment First (APSE) in 2021. She is currently studying at Clemson University to obtain a Ph.D. in Parks, Recreation, and Tourism Management with a concentration in Recreational Therapy.
Currently, Holly is working part-time as a professor at Central Michigan University. She continues to work for TRS to provide support with supervising vocational specialists and recreational therapists.

Megan Groat, BS, CTRS, AIT
CERTIFIED THERAPEUTIC RECREATION SPECIALIST
Megan started working for Transitions in January of 2022. She has been a Certified Therapeutic Specialist since 2019 upon receiving a Bachelors of Science in Therapeutic Recreation from Central Michigan University. She completed a clinical internship at Spaulding, a top ranked Rehabilitation Hospital, located in Boston, MA. During this internship, she gained experience in adaptive sports for people with various disabilities. Throughout her career, she has had the opportunity to work as a Recreation Therapist for a variety of populations including; children with developmental disabilities, individuals with spinal cord and traumatic brain injuries, older adults, and people with psychological disorders. Megan’s Main focus is on Community Integration and Leisure participation to increase independence and quality of life for her clients. Megan has a passion for teaching others about the importance of leisure participation following their injuries. Most recently, Megan obtained a certification in Adaptive & Inclusive Training to better serve her clients working on their fitness goals.

Marisa Colpetzer, BS, CTRS, ATRIC
CERTIFIED THERAPEUTIC RECREATION SPECIALIST
Marisa joined Transitions in July 2020 as a Certified Therapeutic Recreation Specialist. Marisa graduated from Central Michigan University with a Bachelor of Science in Recreation, Parks and Leisure Services Administration with a concentration in Therapeutic Recreation. She has experience in outpatient rehabilitation, acute rehabilitation, as well as home and community setting. She has worked with many different populations including pediatrics and older adults with physical disabilities, individuals with developmental disabilities, at-risk youth, and group dynamics of all ages.
Marisa is certified in Aquatic Therapy and Rehab. She has several years of experience providing Aquatic Therapy in an outpatient rehabilitation setting with various age groups. Through her experience Marisa has developed a true passion for her profession and the populations that she works with.

Grace Justice, BS, CTRS
CERTIFIED THERAPEUTIC RECREATION SPECIALIST
Grace began working for TRS in June 2024. She graduated with a Bachelor of Science in Therapeutic Recreation from Eastern Washington University in 2023 as well as a Bachelor's degree in Outdoor Education from Ohio University in 2021. Grace obtained the Certified Therapeutic Recreation Specialist (CTRS) title in April 2024 and is embarking on her career here with us at Transitions Reintegration Services.
Grace has traveled across several states, including Ohio, Montana, and Michigan, sharing her skills and passion for Recreational Therapy. She possesses a wide range of experience in providing Recreational Therapy services to diverse populations, such as children in psychiatric care, adults and children with disabilities, and individuals with spinal cord and traumatic brain injuries. She is super excited to begin her career with us and continue her dedication to helping individuals achieve independence through engaging leisure activities.

Adeline VanNoller, BS, CTRS
CERTIFIED THERAPEUTIC RECREATION SPECIALIST
Adeline joined TRS in January 2025 as a Certified Therapeutic Recreation Specialist. She earned her Bachelor’s degree in Recreational Therapy from Grand Valley State University and has diverse experience working across various settings. Her background includes providing recreational therapy for older adults in a senior center, completing an internship in Colorado Springs serving children and adults with physical and developmental disabilities, and working in behavioral health with individuals with eating disorders. She is committed to a person-first approach, recognizing the versatility and individualization of care in recreational therapy. With a strengths-based perspective, she strives to empower clients, promote well-being, and enhance their quality of life through meaningful therapeutic interventions

Brooke Fox, BS, CTRS
CERTIFIED THERAPEUTIC RECREATION SPECIALIST
Brooke started working for Transitions in August of 2023. She has been a Certified Therapeutic Specialist since 2020 upon receiving a Bachelors of Science in Therapeutic Recreation from Grand Valley State University. Brooke has a wide range of experience in providing Recreational Therapy services for several populations. Throughout her career, she has worked with at-risk youth in foster care, adults in psychiatric care, older adults with dementia, and individuals with spinal cord and traumatic brain injuries. Brooke has a passion for assisting individuals to gain as much independence in leisure participation and reaching their full potential following their injuries.

Katie VanHekken, BS
VOCATIONAL SPECIALIST & RECREATIONAL THERAPY ASSISTANT
Katie joined TRS in 2021 as a Vocational Specialist and Recreational Therapy Assistant. She has a Bachelor of Science with an emphasis in Therapeutic Recreation degree from Calvin College. Katie brings with her several years of experience working in the rehabilitation industry. Formerly CTRS certified, Katie worked for Hope Network Rehabilitation Services in neuro and spinal cord injury rehabilitation from 1999 to 2007. She was also Bad Ragaz Method aquatics certified and provided assistance in other departments such as occupational and physical therapy in a Rehab Aide capacity. Katie’s strengths as a therapist include establishing rapport with her clients, providing person-centered care, and helping her clients to transition to competitive employment.

Cali Ashby, LMSW
VOCATIONAL SPECIALIST
Cali is a Social Worker by trade and brings with her several years of relevant work experience across a variety of settings, including schools, hospital and rehabilitation facilities. Cali is seasoned in conducting patient assessments and creating treatment plans to address patients’ needs. We are so excited to have her as a Vocational Specialist to assist our clients with achieving work reintegration goals.

Katherine Stotts, BA
VOCATIONAL SPECIALIST & RECREATIONAL THERAPY ASSISTANT
Katie joined Transitions Reintegration Services in 2025 as a Vocational Specialist. She holds a Bachelor of Arts in Psychology with a minor in Neuroscience from Wheaton College. At Wheaton, Katie concentrated her research on topics related to memory and post-traumatic stress disorder, developing a strong foundation in trauma-informed care and cognitive functioning. She also contributed to research in collaboration with Harvard and Duke psychology programs, with an emphasis on client-centered methodology and evidence-based inquiry. Katie has clinical experience supporting children, families, and older adults, including individuals with neurocognitive disorders in memory care settings. She takes a holistic, person-centered approach to vocational rehabilitation, addressing emotional, cognitive, and psychosocial factors that impact long-term outcomes. Her clinical strengths include therapeutic rapport building, functional goal-setting, and the delivery of individualized vocational interventions.

Diane Elizondo, AS, COTA
Diane Elizondo is a Certified Occupational Therapy Assistant (COTA) with over 14 years of experience, specializing in geriatrics and pediatrics. Since earning her Associate Applied Science Degree in 2011, she has worked across a wide range of settings, including skilled nursing facilities, home care, outpatient clinics, and pediatric therapy centers.
Throughout her career, she has provided care for individuals recovering from auto accidents, brain injuries, spinal cord injuries, strokes, and amputations. Her expertise includes assisting clients with ADLs and IADLs, implementing play-based therapy for children, using therapeutic modalities, supporting motor skill development, and working with clients experiencing dementia.
In July 2025, she joined TRS, bringing her deep clinical knowledge and compassionate approach to care. Known for her infectious personality and ability to build strong, lasting relationships, she quickly earns the trust of her clients and their families. Her passion for occupational therapy is evident in every session, where she combines professionalism, creativity, and empathy to help clients regain independence and improve their quality of life.tion goes here
